Using WordPress 2.8.4 and the latest 3.1.3 of the Custom Anti-Spam plugin.
According to the plugin options page, I have the GD and FreeType library, and get_option works. I have MediaWiki on this site, which successfully creates files and similar actions, so I don't suspect file permission problems. Yet for some reason, I can never get the image to show up when the plugin is installed.
When I choose to view the image, I get, "The image “[url omitted]” cannot be displayed, because it contains errors." When I have sound enabled, the sound file also fails to render.
I have tried almost every combination of settings, but am at a loss to figure this out. This is a completely fresh install of the plugin, with no custom words or files. I will leave the plugin turned on for a while so that you can see.

You might have a plugin or other server setting that is generating some blank output or response at the top of every page (or at least every script). While HTML is forgiving of this, a script generating an image would have problems. Unfortunately, without digging deeper I don't have any other suggestions, but you could talk to your web host about that.
